ROGERIO LILENBAUM, MD: I think the biggest value of neoadjuvant therapy is the potential for delivering meaningful doses of chemotherapy, which is probably more likely to happen in the preoperative setting than it is in the postoperative setting. I have to say, though, this approach is a difficult concept for both physicians and patients. And it's one that, in my opinion, probably will really never catch up as a standard option. Patients and surgeons are anxious and eager to go to surgery. There is always that possibility that patients may progress during the induction chemotherapy, although it wasn't really something we saw with any frequency in the SWOG 9900 trial.16 I have not used induction chemotherapy very often in my practice. I will however use this approach in patients who have borderline resectable tumors. For tumors that abut the mediastinum or for tumors that are very large or that extend to the chest wall which would require a major resection ; , I would use neoadjuvant therapy. Not because I believe that it provides a better opportunity for survival compared with adjuvant treatment, but to downstage the tumor and allow for a cleaner and easier surgical resection. H. JACK WEST, MD: Are there any current trials that could define a role for the neoadjuvant approach in early-stage NSCLC? ROGERIO LILENBAUM, MD: There is a trial NATCH ; led by Dr Rosell in Spain looking at neoadjuvant versus adjuvant therapy, which I think will shed some light on this issue Figure 5 ; . I don't know where this trial is in terms of accrual or when it might be presented, but I think that it's an important study. There is also a study led by Dr Belani UPCI 04-094 ; on these 2 options Figure 5 ; . I believe these studies will give us some additional information on this issue. H. LIPID LOWERING EFFECT OF RENAGEL, A NOVEL PHOSPHATE BINDER J. Bommer, European Renagel Investigators group * Med versittsklinik Heidelberg Reduction of serum lipids is recommended in most dialysis patients. In a randomized multicenter study, the lipid lowering effect of Renagel a novel phosphate binder has been evaluated. Patients: 114 hemodialysis patients, treating on thrice weekly dialysis for 3 months were enrolled. Patients with poorly controlled diabetes or hypertension, severe motility disorders of gastrointestinal tract, chronic inflammatory status or infections, and malignancy were excluded. Patients had a mean age of 57 14 years and had been treating on dialysis for mean of 66 67 months. Sixty-three percent n 72 ; were male. Patients were randomized to receive either Renagel or calcium carbonate. After a baseline determination of serum lipid levels, patients were treated with doses of sevelamer or calcium carbonate, titrated according to serum phosphate levels. Results: Median lipid levels are given in the table. Renagel Ren ; decreased total cholesterol TC ; by a median of 20% . LDL cholesterol LDL-C ; decreased by a median of 27 %, HDL cholesterol HDL-C ; and triglycerides TG ; did not change and reserpine.

As a precaution, samples should be stored frozen at -20 # C until assay or assayed soon after collection. Repetitive freeze thaw cycles, which may challenge the integrity of the samples, should be avoided and restasis.
